1. HOW TO ENTER/ WINNER SELECTION: Download the COSI app (Google Play or Apple App Store) and complete the COSI Smart Challenge or register on-site upon request with a Guest Services Team Member or mail a 3x5 card with your name and email address to COSI / Smart Challenge, c/o COSI Marketing Department, Attn: Marketing Director, 333 West Broad Street, Columbus, OH 43215. Each mailed entry must be sent separately in a hand-addressed, stamped envelope. One entry per household will be accepted, regardless of the method of entry. Any entries received in excess of this maximum of one (1) entry will be disqualified. No photocopied or mechanically reproduced entries will be accepted. No responsibility is assumed by Sponsors (as defined below in Section 8) for lost, late, illegible, incomplete or misdirected entries. All mailed in entries must be postmarked by February 19, 2019 and received by February 26, 2018. The Giveaway shall be subject to these Official Rules, and by entering, all participants agree to be bound by the terms and conditions herein. False, fraudulent or deceptive entries or acts shall render entrants ineligible. All federal, state and local rules and regulations apply. Void outside of the Eligibility Area (as defined below in Section 3) and where prohibited or restricted by law. Entries must be legible for eligibility. The contest period is from January 16, 2019 10am to 5pm on February 18, 2019.

Odds of winning depend upon the number of eligible entries received. Prize winner will be randomly selected from all eligible entries by COSI on or about February 26, 2019, and the winner will be contacted by COSI. COSI will notify the potential winner as specified in Section 5 below.

2. PRIZES: One Prize will be awarded:

Grand Prize (1 awarded): One COSI ultimate family membership, good for one year from time of activation ($329 value). May be used by the winner or given as a gift. Prize recipient is responsible for purchase of annual renewals (if desired). Grand Prize does not include parking, store or café credit and/or admission to special, up-charge exhibitions, other restrictions may apply. Prize winner will also receive a $100 gift card to Easton.

No responsibility or liability is assumed by Sponsors. Total value of prize package: Grand Prize, $429.00

3. ELIGIBILITY: The Giveaway is open only to legal residents of the United States (except residents of Rhode Island, Puerto Rico, and all U.S. Territories and Possessions) and District of Columbia (the “Eligibility Area”) who are eighteen (18) years of age or older. Employees of Sponsors and their respective subsidiaries, affiliates, divisions, distributors, suppliers and advertising, promotional or judging agencies (collectively, “Releasees”) and immediate family members of such employees (parent, child, sibling, spouse) and persons living in the same household of such employees (whether related or not) are not eligible to win.

4. GENERAL: No substitution of Prize is offered and non-cash prizes may not be redeemed for cash value. Void wherever prohibited or restricted by law. Prize winner will be solely responsible for paying all applicable federal, state and local taxes on Prize. No groups, clubs or organizations may enter or win Prizes. All entrants, as a condition of entry, agree to be bound by these rules and the decisions of COSI which shall be final and binding. Sponsors reserve the right to change or discontinue this promotion at any time should any cause beyond the control of the Sponsors affect the administration, security, fairness, integrity or proper operation of the Giveaway, or for any other reason the Sponsors deem appropriate, at their sole and absolute discretion. In the event this Giveaway is cancelled, if possible, the Sponsor will conduct a random drawing to award the prizes from among all eligible, non-suspect Entries received prior to the time of the action or event warranting such cancellation. Except for mail-in entries, Entrant’s name, telephone number, and email address supplied at entry will be added to Sponsors’ marketing databases and may be shared by the Sponsors with additional parties.

5. WINNER NOTIFICATION: The potential winner will be notified by email by the email address provided at entry. If any required documents are not returned within the required number of days, or if email is returned as undeliverable without forwarding address, then Prize may be forfeited and COSI may select an alternate winner.

6. RELEASE OF LIABILITY: All entrants, as a condition of participation, and the winner and their guests, by acceptance of a Prize, agree to release Releasees from any and all liability, claims or actions of any kind whatsoever for injuries, damages or losses to persons and property which may be sustained in connection with participation or inability to participate in this Giveaway, or the receipt, ownership or use of the Prize. The Sponsors do not make any representations or warranties of any kind concerning the appearance, performance or safety of any Prize awarded. Upon request, winner must consent (and confirm such consent in writing) to the use of their name, voice, picture or other likeness without compensation for promotional purposes as determined by Sponsors, except where prohibited by law, and such individual hereby releases Releasees from any liability with respect to such use.

Releasees are not responsible for: any incorrect or inaccurate entry information; human errors; theft, tampering, destruction or unauthorized access to, or alteration of, entries; data that is processed late or incorrectly or is incomplete; typographical, printing or other errors; and late, lost, stolen, mutilated, misdirected or postage-due mail.

Releasees shall not be liable to winner or any other person for failure to supply a Prize or any part thereof, by reason of any acts of God, any action(s), regulation(s), order(s) or request(s) by any governmental or quasi-governmental entity (whether or not the action(s), regulation(s), order(s) or request(s) prove(s) to be invalid), equipment failure, terrorist acts, earthquake, war, fire, flood, explosion, unusually severe weather, hurricane, embargo, labor dispute or strike (whether legal or illegal), labor or material shortage, transportation interruption of any kind, work slow-down, civil disturbance, insurrection, riot, or any other cause beyond Releasees’ sole control.

7. WINNER IDENTIFICATION: For identification of the Grand Prize Winner, send a self-addressed stamped envelope before March 28, 2019 to COSI / Smart Challenge, c/o COSI Marketing Department, Attn: Marketing Director, 333 West Broad Street, Columbus, OH 43215.
