What forms of ID are accepted to qualify for Family Access Memberships?

To join as a Family Access member, families must show a photo ID and current participation in WIC, Ohio Directions, or Medicaid (from Ohio).
When purchasing online or over the phone, one of the above forms needs to be emailed or faxed (614.629.3150) to COSI.
